OHRP shows up. FDA asks for an inspection packet. AAHRPP wants to see your audit trail. The current "audit log" is a folder full of emails and a search of who-changed-what-when across five systems.
How it works
Study creation, submission, reviewer assignment, vote, amendment, AE report, CR scheduling, meeting outcome, determination signing — every state change writes an entry.
Filter by action type, user, date range, or study. Drill from a study back to every action that touched it.
Need the last 6 months of activity for an inspection? Pick your filter, click Export — CSV downloads with full metadata.
Audit entries are append-only. IP addresses are hashed (never stored raw). Retention is per-institution policy with default 7 years.
